One bed, two wives: Interested?

This gentleman come into my practice with a complaint of erectile dysfunction for 2 months. He always took aphrodisiacs “herbal” to strengthen his erections these days. And now he wants some medications to make him stronger erections. Before he came in, I saw a lady in my waiting room sitting beside him. I asked her, ” Ibu tidak ikut masuk? Maam, do you want to come in?” I thought she is his wife. ” Tidak usah dok, saya tunggu di sini saja sambil nonton tivi. No, thanks, doc, I just wait here and watch television”

The next visit, I saw another older lady coming with that gentleman patient. So I asked him when he had entered the consultation room. “Tidak sama Ibu, pak? Dont you come with your wife, Sir?” The man smiled widely, ” Hehe, ini yang pertama dok. Yang dulu yang kedua. Hehe, this is my first wife, doc. The lady you saw last visit is my second wife” I was startled to hear that and I couldnt stand not asking, ” Tinggalnya di satu rumah, Pak? Do you live in the same house?” To my surprise, he answered, ” Hehe, malah satu ranjang bertiga, dok. Hehe, doc, one bed for three of us” And I was very ‘impolite’ but curious, “Wah, berarti kalau ‘main’ bertiga langsung bareng dong? So you do threesome?” The man laughed, “Iya dok, sudah biasa. Yes, doc, it is a usual situation. We do it threesome. ”

Haree genee, ternyata masih ada ya??? These days.. we still have that kind of ‘peaceful polygami’????

Masa Subur? Bagaimana mengetahuinya?

Bila ingin mendapatkan anak, maka berhubunganlah pada saat masa subur. Tapi bila belum ingin mendapatkan anak, maka hindarilah berhubungan pada masa subur.

Ada beberapa cara menghitung/ memprediksi masa subur:
1. Sistem kalender
Cara ini bisa digunakan bila siklus menstruasi teratur antara 28 hari sampai 35 hari.
Prinsipnya adalah:
a. Ovulasi (pengeluaran sel telur dari indung telur) terjadi 14 hari sebelum menstruasi yang akan datang.

b. Sperma dapat bertahan hidup sampai 48 jam setelah ejakulasi.

c. Sel telur dapat bertahan hidup selama 24 jam setelah ovulasi.

Cara menghitung dengan sistem kalender:

(i) Catat tanggal pertama haid, minimal 6 kali menstruasi

(ii) Hari pertama masa subur = Jumlah hari terpendek – 18

(iii) Hari terakhir masa subur = Jumlah hari terpanjang – 11

Contoh:

Siklus terpendek 28 hari dan siklus terpanjang 32 hari (mulai hari pertama haid sampai haid berikutnya)

Perhitungannya : 28 -18 = 10 dan 32–11 = 21. jadi masa suburnya adalah mulai hari ke-10 sampai ke 21 dari hari pertama haid. Pada masa ini suami isteri tidak boleh berhubungan intim. Bila ingin berhubungan intim, disarankan memakai kondom.

Cara KB dengan sanggama terputus tidak disarankan karena angka kegagalannya cukup tinggi yaitu 25-30% , karena dikhawatirkan ada sperma yang tetap lolos , bisa berenang sampai saluran tuba falopii (saluran telur) dan bisa membuahi sel telur.

2. menilai peningkatan suhu badan, biasanya suhu badan meningkat menjelang dan sesudah masa ovulasi karena pengaruh hormon progesteron.

3. menilai lendir rahim. Hormon estrogen mencapai puncaknya pada saat ovulasi terjadi dan memengaruhi lendir rahim. Menjelang ovulasi biasanya lendir rahim jadi agak encer dan bila diraba dengan dua jari membentuk benang dan berwarna bening.

4. dengan Alat Tes Ovulasi. Saat ini juga sudah dijual bebas diapotik atau toko obat alat untuk menilai masa subur yaitu Test prediksi masa subur atau ovulasi. Alat test ini bekerja dengan mengukur kadar hormone LH yang dihasilkan saat ovulasi. Alat ini digunakan seperti test kehamilan tetapi tentu saja harganya lebih mahal.

Vaginismus: Sulit Masuk karena ‘Sempit’?

Vaginismus? Apa itu?

Vaginismus adalah kontraksi involunter (diluar kehendak sadar) dari otot vagina dan otot dasar panggul. Otot tsb secara refleks menegang/ mengejang sehingga tidak bisa dilakukan penetrasi penis, atau obyek lain seperti tampon, spekulum (alat medis untuk melihat dalamnya vagina).

Ketegangan otot ini menimbulkan rasan nyeri, rasa terbakar, perih, terasa nyeri tajam. Vaginismus bisa timbul secara primer (seumur hidup, tanpa diketahui penyebabnya), atau sekunder (tadinya normal, lalu setelah ada pemicu/ penyebab, baru timbul vaginismus). Bisa timbul secara global/ umum (terjadi pada semua situasi dan pada semua obyek yang akan dimasukkan ke vagina), atau situasional (hanya terjadi pada situasi tertentu, misalnya hanya pada satu partner tertentu dan lainnya tidak, atau hanya timbul pada hubungan seksual, tapi tidak timbul saat akan memasukkan tampon atau pemeriksaan vagina dengan jari dokter kandungan/ VT).

Wanita yang vaginismus sering menganggap bahwa mereka ‘terlalu sempit’, sehingga vagina harus diregangkan. Ini menimbulkan bayangan bahwa hubungan seks atau penetrasi apa pun akan menimbulkan nyeri. Padahal wanita yang vaginismus tidak perlu meregangkan vagina, mereka justru perlu belajar mengontrol otot sekitar vagina, ini bisa dilatih dengan exercise.

How long can sperm survive after ejaculation?

Research has shown that some sperm may survive in the Fallopian tubes up to 7 days, with the average amount of time being 3 to 4 days.

The sperm survive much longer once they have traveled through the cervical opening into the woman’s body which is a good reason to try and elevate your bottom in order to encourage the sperm to travel, with the help of gravity, to your cervix more quickly and in greater quantities.

Theories also suggest that the contractions associated with orgasms pull sperm from the vagina to the cervix, where it’s in better position to reach the egg.

Sperm, in the vagina, can only survive about six hours due to the acidic vaginal secretions. The cervical mucus present when ovulation is near, is more alkaline and more hospitable to sperm. The egg white consistency of the cervical mucus helps the sperm move more easily through the vagina to the cervix, increasing the chance that the sperm will be in the correct location for fertilization to occur.

If the woman’s cervical mucus is not as hospitable to the sperm as it should be, not enough egg white consistency, she may need to use a lubricant to help. Pre-Seed personal lubricant is the only lubricant on the market which does not harm sperm. The consistency and pH is as close to natural cervical mucus as you can get.

Wanna Be Starting Something?

If you have to start something, you often have difficulty to the beginning. Are you the one who experience it in the whole wide world? of course not. Every first step is very difficult to make.

Actually it is very simple but it is we who make it complicated by asking a lot of thing. Thinking too much about what happen if we do the beginning. We think we have to anticipate everything we possiby face in the process. But after a lot of thingking, then we stop too long for thinking only.

Thinking is good, but thinking too long and too much is not good at all. Think just long enough to start and to do the beginning. After the start we can do the action and the thinking can be done later.

Thinking too long is for the negative thing, such as before we used narcotics, cheating to our spouses. That needs a lot of thinking.. haha.. But for the daily routine, for our work, for the good practical things, you dont have to think too long. Such as going to the gym, cooking for your children, writing your assignment, telephoning your client, that doesnt need thinking too much and too long. Just start picking your telephone and dial the number of your prospect. Or turning on your PC and do your homework, make a report that your boss has asked you several times. When you feel very lazy to do something, then it is the time you lift up your butt and walk and grab your brush and clean the toilet, or broom your bedroom, or typing the report. Then you will clean all your room and toilet, also ‘clean your heart from laziness” and it will motivate you to do something ‘more important’. Some motion and movement is necessary at the beginning. So dont be afraid to ‘get busy’ and to ‘look busy’ at the first time. Just ‘get busy’ at the beginning, then you can think the better things to do next, or you can think the better strategy to continue throughout the day. It will give you a good impact to start your successful day.

If you want to tidy up the home, then lift some broom and brush. If you want to sell something, then speak to your colleague about the prospect. If you want to repair the broken car, then walk to your car and lift up the hood. Dont just sit here reading this note. This note can become silly when you just read it too long. So now, get up and do something. Get busy.

Eureka!! ditemukan “Vaksin” untuk kanker prostat!

Perusahaan biotek Dendreon (di Seattle) menemukan vaksin untuk kanker prostat, yang dinamakan Provenge. Prosedurnya: darah dari pasien diambil, lalu dikirim ke lab. Teknisi mengekstrak sel imun yang disebut APC (Antigen Presenting Cell) dari sampel darah tsb. Dari sini, Dendreon mengombinasikan sel imun tsb dengan protein yang ada di permukaan sel kanker prostat. Ditambahkan juga bahan pendongkrak imunitas (immune booster). APC akan bekerja bila bercampur protein sel kanker dan immune booster. Lalu protein kanker prostat ini akan pindah ke permukaan sel yang sudah “teraktivasi”. Kemudian, APC yang sudah teraktivasi ini dimasukkan kembali ke aliran darah pasien kanker prostat.

Selama ini, kebanyakan kanker sulit diterapi karena sistem imun tidak bereaksi terhadap sel kanker. Kan biasanya bila ada infeksi atau virus, sistem imun kita akan mengenalinya sebagai benda asing. Tujuan vaksin Provenge ini adalah untuk membuat sistem imun mengenali sel kanker dan menghancurkannya. Pada saat APC yang teraktivasi kembali ke darah pasien, APC akan kontak dengan sel T (sel darah putih yang ’seek and destroy’ benda asing). Lalu sel T tersebut juga akan teraktivasi. Sel T akan mengenali sel kanker/ tumor sebagai ‘penjahat’ atau benda asing, dan akan memburu dan menghancurkan sel kanker (benda asing) tersebut.

Vaksin diberikan secara intravena sebanyak tiga dosis/ tiga kali dalam beberapa mingu. Tapi harganya masih mahal, karena labor-intensive, dan patient-specific, harganya sekitar 50-75 ribu USD. Sementara itu, sambil menunggu FDA announcement, saham Dendreon melonjak 19%. Wow!!

Seringkali di filem-filem, tampaknya bila sepasang suami istri bertemu, mereka sangat mesra satu sama lain, dan dengan mulusnya berlanjut ke hubungan intim yang menggairahkan. Tetapi pada kenyataan sehari-hari, kita yang sudah menikah bertahun-tahun, belasan tahun atau lebih, tahu bahwa hal itu tidak selalu terjadi semudah itu. Seringkali hal-hal di luar seks itu bisa menghambat gairah dan terjadinya hubungan intim yang memuaskan, misalnya tekanan di tempat kerja, habis dikritik habis-habisan saat meeting, dikecewakan rekan sekerja yang janjinya kosong melulu, atau ibu rumah tangga yang capek badan dan capek hati karena pekerjaan terasa tidak habis-habisnya dan tidak kelihatan hasilnya.

Lalu, bagaimana solusinya? Coba bikin janjian dengan pasangan anda, kapan anda mau bercinta dengannya. Buat persiapan yang perlu untuk bisa menimbulkan suasana yang mendukung.
